Homocysteine

Table of contents

Homocysteine is an amino acid your body makes during normal protein and B-vitamin metabolism, measured in blood as a marker of methylation status and cardiovascular health. In most healthy adults it sits between 5 and 15 µmol/L, with lower values inside that band generally seen as more favorable.

What homocysteine measures

Homocysteine is an amino acid produced when your body breaks down methionine from dietary protein. Clearing it depends on adequate folate (B9), vitamin B12, and vitamin B6, so the level acts as a functional readout of your B-vitamin status and methylation capacity. Because elevated concentrations have been linked in research to vascular and cognitive outcomes, clinicians sometimes use it as a supporting marker of cardiovascular and brain health.

What high or low levels mean

High homocysteine most often reflects low folate, B12, or B6 intake or absorption, but it also rises with reduced kidney function, hypothyroidism, smoking, certain medications like methotrexate, and inherited conditions such as MTHFR variants or homocystinuria. Elevated levels have been associated with higher cardiovascular and thrombotic risk, though correcting an underlying deficiency and rechecking usually clarifies the picture. Low homocysteine is generally not a health concern and typically signals good nutrient status. Follow up with a clinician if your result is above the lab range, if you have kidney disease, or if you have neurologic symptoms, since interpretation alongside B12, folate, and kidney markers guides next steps.

Homocysteine FAQ

Can I lower homocysteine naturally?

Often yes. Improving folate, B12, and B6 intake through leafy greens, legumes, and fortified foods, and correcting any deficiency, commonly brings elevated levels down. Reducing smoking and treating thyroid or kidney issues also helps.

Should I fast before a homocysteine test?

Many labs ask for an 8 to 12 hour fast because recent protein intake can transiently raise levels. Follow your ordering clinician's specific instructions, and try to test under consistent conditions each time.

Does high homocysteine cause heart disease?

Elevated homocysteine is associated with higher cardiovascular risk, but the link is not fully settled as a direct cause. It is best seen as one supporting marker read alongside your other cardiovascular and nutrient results.

Read alongside vitamin B12, hs-CRP, and non-HDL cholesterol.
